The most interesting indications for mitral valve surgery in severe mitral valve regurgitation that few clinicians are aware of:

Class IIa indication for surgery in ASYMPTOMATIC patients with normal heart function, or in the presence of new onset atrial fibrillation or pulmonary hypertension, if the likelihood of repair is greater than 95% and the expected mortality is less than 1%.
