What is a Double Heart Bypass Surgery?

Double bypass surgery, or coronary artery bypass grafting (CABG), restores blood flow to the heart muscle.

During surgery, your surgeon creates a new route for blood flow around the blocked arteries using healthy blood vessels (grafts) taken from other parts of your body, such as the chest wall, leg and arm.

The number of bypasses depends on the number of heart arteries affected. For example, a double bypass reroutes blood around 2 blocked arteries

Why You Might Need Double Bypass Surgery

A double heart bypass operation is used to treat coronary artery disease. This type of heart disease occurs when the arteries that supply blood to the heart muscle become narrowed or blocked due to a buildup of fatty deposits, known as plaque.

When these arteries become too narrow, your heart muscle may not receive enough blood with oxygen. This lack of oxygen in the blood supply is particularly prevalent during exercise or periods of stress.

You may experience heart disease symptoms such as:

  • Chest pain (angina)
  • Shortness of breath
  • Fatigue

In more severe cases of coronary heart disease, a complete blockage can cause a heart attack.

Possible Complications & Risks of Double Coronary Bypass Surgery

While double coronary artery bypass surgery is a standard procedure, like all major surgeries, it carries risks. Some are procedural, while others depend on individual patient factors.

General Risks

  • Stroke: Occurs in approximately 1% of patients during or shortly after surgery.
  • Mortality: Risk varies based on age, medical history, and overall health status.
  • Advanced age: Older patients face an elevated risk of stroke, complications, and prolonged recovery.

Patient-Specific Risk Factors

Certain pre-existing conditions increase surgical risk:

  • Frailty (reduced physical strength and functional capacity)
  • Poorly controlled diabetes mellitus
  • Chronic obstructive pulmonary disease (COPD)
  • Co-existing illnesses such as liver disease
  • Peripheral arterial disease (PAD)
  • Previous heart attack with permanent heart muscle scarring

Common Postoperative Complications

  • Pleural effusion: Fluid build-up in the chest cavity affects approximately 11% of patients and may extend hospitalisation.
  • Extended surgery time: Prolonged use of the heart-lung machine and aortic cross-clamping increases the risk of complications.
  • Low body mass: Associated with a higher risk of pleural effusion and other complications.
  • Progressive angina: Chest pain that recurs or persists after surgery, not to be confused with postoperative pain
  • Deep sternal wound infection: More common with traditional open heart surgery than minimally invasive approaches.
  • Vein graft failure (VGF): VGF is common and increases the likelihood of needing another procedure, but does not typically raise the risk of heart attack or death.

Suitability for TCRAT

TCRAT is typically suitable for patients who:

  1. Have multi-vessel coronary artery disease
  2. Are clinically stable
  3. Have a favourable anatomy that allows access through a small incision

Your surgeon will confirm suitability using angiograms and CT imaging before recommending this procedure.

1. Minimally Invasive Direct Coronary Artery Bypass (MIDCAB)
  • Typically for single-vessel, like the left anterior descending artery(LAD)

  • Small left-chest incision 

  • Heart keeps beating, no heart-lung machine

2. Totally Endoscopic Coronary Artery Bypass (TECAB)
  • Fully robotic, tiny chest incisions (performed using robotic arms)

  • Single or sometimes double bypass (depending on the arteries involved)

  • Requires specialised equipment and training (only certain surgeons and centres can perform)

3. Hybrid Procedure

Minimally invasive bypass that combines surgery and catheter-based treatment)

  • Reduced in-hospital major complications

  • Fewer blood transfusions and chest drainage

  • Can avoid complete open-chest surgery in multi-vessel disease (less invasive recovery) 

What to Expect from Double Bypass Heart Surgery

Heart bypass surgery can be performed using minimally invasive keyhole techniques or traditional open-heart surgery. The keyhole approach offers several significant advantages, including smaller incisions.

Here is what you can expect from a double CABG procedure.

1. Preparation

Surgery begins in the operating room under general anaesthesia. A breathing tube is placed, and monitors track your heart rate and blood pressure throughout the procedure. Additional monitoring equipment ensures a precise assessment of your heart function. 

2. Access to the Heart
  • Keyhole approach: Your surgeon makes several small incisions between the ribs. Using a specialised camera and precision instruments, the heart is accessed without opening the breastbone. This results in less pain, faster healing, and a lower risk of infection.

  • Traditional open-heart approach: A single incision is made down the centre of the chest, and the breastbone is broken open to access the heart.

3. Preparing the Bypass Vessels

Your surgeon uses healthy blood vessels from other parts of your body to create new pathways around the blockages. The blocked arteries themselves remain in place. Common sources include:

  • Internal mammary artery (from inside the chest wall)
  • Radial artery (from the forearm)
  • Saphenous vein (from the leg)

Arterial grafts typically provide better long-term results than vein grafts, so arteries are the preferred choice when possible.

4. Protecting Your Heart During Surgery

You receive anticoagulant medication (a blood thinner) to prevent blood clotting.

  • With keyhole surgery, it is safe to perform on a beating heart, eliminating the need for a heart-lung machine and reducing potential complications.  

  • In traditional open-heart surgery, a heart-lung machine temporarily takes over your heart and lung function. Throughout the procedure, your expert surgical team carefully monitors blood pressure and circulation.

5. Creating New Blood Pathways

Your surgeon connects the healthy blood vessels to your coronary arteries, routing blood around the blockages. The other end of each graft is attached to the aorta (main artery) or another suitable vessel. This restores optimal blood flow to your heart muscle.

6. Ensuring Proper Heart Function

If a heart-lung machine is used, your heart is restarted and normal blood flow is restored. The surgical team carefully checks each bypass graft to ensure proper blood flow and confirms there is no bleeding.

7. Closure
  • Keyhole surgery: Only small incisions require closing, typically with dissolvable stitches. No breastbone wires are needed.

  • Traditional surgery: The breastbone is secured with permanent stainless steel wires, and the chest incision is closed with dissolvable stitches.

After Your Surgery

The process is handled with the utmost care. You will be transferred to the Intensive Care Unit (ICU) for close monitoring during early recovery. An experienced healthcare team will support you as you begin to recover.
