In short, mitral valve repair surgery treats a leaking mitral valve and restores normal blood flow through the heart. Instead of replacing the valve, the surgeon repairs your own valve so it can close properly again.

What Is the Mitral Valve?

  • They open and close to ensure that blood flows in one direction, from the left atrium to the left ventricle.
  • When the left ventricle contracts to pump blood around the body, the valve closes tightly to prevent backflow into the atrium.

Mitral regurgitation occurs when the valve leaks, often due to mitral valve degeneration that weakens the valve tissue and the chordae. The chordae are string-like cords that attach the valve to the heart wall, like thin, flexible tubes, helping the valve leaflets move properly.

  • Healthy chordae prevent the leaflets from turning the wrong way.
  • Degeneration can cause leaflets to fail to seal, sometimes affecting one or both.
  • Ruptured chordae can lead to severe, rapid leakage, while slower degeneration may cause chronic leakage.

When Mitral Valve Surgery Is Needed

We usually recommend mitral valve repair surgery when:

  • The mitral valve leak (mitral regurgitation) is severe, even if you don’t notice many symptoms
  • The left atrium starts to enlarge, or you develop atrial fibrillation or pulmonary hypertension
  • The valve is highly repairable, with a good chance of long-term success (over 95%) when the valve anatomy is suitable and surgery is performed in experienced centres.

How a Keyhole Mitral Valve Repair is Performed 

If you have a leaking mitral valve, repair is often the preferred treatment. In many cases, this can be carried out using keyhole (minimally invasive) surgery.

Keyhole mitral valve repair is a minimally invasive procedure that carries fewer risks than traditional open-heart surgery. It is performed through a small incision on the right side of the chest, between the ribs, and does not require cutting any bone.

During a mitral valve repair, we may use one or more of the following techniques:

1. Replacing damaged valve strings (chordae)

If the fine “strings” that support the valve are torn, they can be replaced with durable Gore-Tex® chords, allowing the valve to open and close normally again.

2. Repairing the valve leaflet

Occasionally, a small abnormal section of the valve leaflet is carefully removed (triangular resection) to help the valve seal properly and stop the leak.

3. Stabilising the valve with a support ring

Once the leaflet is repaired, a soft ring is stitched into place to restore and maintain the normal shape of the mitral valve opening. This helps ensure the valve continues to work effectively over time.

After surgery under general anaesthetic, you may spend a short time in the intensive care unit, where your vital signs are carefully monitored.
